Distinguish among the different types of pain
Acute or chronic pain:
- List etiology if known (e.g., trauma, cancer, post-procedural, post-thoracotomy, chronic with psychosocial dysfunction)
- Document site and laterality
Central pain syndrome: list etiology or underlying cause, if known.
Phantom limb syndrome: document with or without pain.
